Early Learning Centers will need the following information prior to start of classes.

Information required for application should be brought with you at the time of registration.

Registration/admission record (provided in packet)

Financial Contract - Registration & Supply fee will apply.

Physical and Medical Release form signed and dated by physician  annually (provided in packet)

Immunization Record (all immunizations must be current)

All About Me - a little about your child.

Emergency Information Card - child and contact information
